Information For Individuals and Families

Notice: Information Concerning Providers In An Adverse Action
Licensure rules require that the DODD post a public notification on its website when a licensed facility has been put in a suspension of admissions at least two times within the previous eighteen months. In accordance with that requirement, please note:

The DODD works with County Boards of DODD and other related agencies to ensure the availability of supports to help people live as they choose, promote their health, safety and welfare, and, assist families of individuals in reaching their goals. This website is designed to assist individuals and families in finding their way through the many opportunities available within a fairly complex system of services and supports.

Through advocacy efforts, specialized services, and a network of statewide resources, an effective service plan may be developed for each individual and family served. In addition, the DODD works to provide these needed services in a fair and equitable manner, in light of available funding.

The DODD also seeks to provide opportunities for people to control available resources and make choices about their healthcare needs. We invite you to contact us if you have any questions that remain unanswered after visiting this site. Contact: Peggy Martin, Family Advocate, at peggy.martin@dmr.state.oh.us or call (614) 466-8706.
