Registration and Certification for Employees of County Boards
Who needs to have registration or certification issued by ODMRDD?
Any individual employed by a County Board of Mental Retardation and Developmental Disabilities (CBDODD) in a position that includes directly providing, or supervising persons who directly provide, services or instruction to or on behalf of individuals with mental retardation or developmental disabilities, must hold the appropriate certification, registration, or license. Employees of entities that contract with county boards of mental retardation and developmental disabilities to provide Adult Services, Early Intervention, Investigative Agent, or Service and Support Administration services for individuals with mental retardation and developmental disabilities are subject to the same requirements. This obligation is delineated in ORC 5126.25.
How do I apply for registration or certification?
Complete and submit an Application for Registration and Certification (form DMR-1137) to ODMRDD. Make certain to include the required supporting documentation (e.g., college transcripts, original seminar attendance verification forms) and to sign the application. Employees of CBMRDDs or entities that contract with them must obtain the Superintendent's signature.

Is there an application fee?
Yes; the fee schedule is delineated in OAC 5123:2-5-06. The amount of the fee depends upon the credential requested. The fee may be paid by check or moneyorder payable to "Treasurer, State of Ohio." The fee is non-refundable. Applications submitted without the appropriate application fee will be returned to the applicant.
How long will it take for ODMRDD to process my application for registration or certification?
The Office of Certification strives to process Applications for Registration and Certification within 30 days of receipt. Occasionally, due to seasonal variations in the number of applications submitted, processing may take longer than 30 days. If 60 days have elapsed since your application was submitted to ODMRDD and you have not received your credential, check with the individual who monitors registration and certification for your County Board of DODD, or contact the ODMRDD Office of Certification at (614) 995-7024.
When will my initial credential be effective?
The effective date of an initial credential is the date ODMRDD receives a complete application, including all required documents, and the appropriate application fee.
Can a County Board of DODD hire an employee and bill for Medicaid services provided by that employee prior to the date the employee actually receives a credential from ODMRDD?
In accordance with ORC 5126.25, a County Board of DODD may employ an individual, provided that the individual has applied for the credential, the individual qualifies for the credential, and ODMRDD has not denied the application for the credential.
What do I have to do to renew my credential?
That depends on the level and grade of credential you hold. Individuals holding Temporary Grade credentials must make progress toward completing the requirements for the Provisional grade credential as specified in the applicable rule. Individuals holding Provisional grade and Professional grade credentials must complete ODDODD-approved continuing professional development. The number of units of continuing professional development required for renewal is specified in the applicable rule.
How far in advance do I have to submit my application for renewal to ensure that my credential does not lapse?
Provided ODMRDD receives a complete application (including all required documents and the appropriate application fee) prior to expiration of the current credential, the renewal credential will be effective on the date the current credential expires. When a renewal application is received after the credential has expired, the renewal is effective on the date ODDODD receives a complete application.
The rules containing the standards for registration and certification specify that the applicant must make progress toward completing the requirements for the Provisional grade credential during the effective period of the Temporary grade credential. There is no provision that allows for deviation from this requirement.
What do I have to do to renew my Provisional Grade or Professional Grade credential if it has lapsed?
That depends upon when the credential lapsed and what type of credential you held. If your credential has lapsed for less than one year, it may be renewed upon completion of all renewal requirements.
