Nursing Continuing Education

The DODD is approved by the Ohio Board of Nursing to approve Continuing Education for nurses. The forms supplied must be completed according to the specific directions provided. Original forms and two copies must be submitted at least 6 weeks in advance to:

With these forms you must also submit a copy of the brochure or an outline of the presentation, a copy of the evaluation that will be used, and a copy of the certificate you intend to provide at the completion of the program.  Please note the instructions on page two of the forms are very specific about what must be included on the certificate. After Karen receives these, she will confirm the submission by sending you a postcard.  If anything is missing from your packet it will be noted on this postcard.  The application will not be processed until that material is received.  She will call you with an approval number once the review has been completed.

After the actual CE program, return to Karen a copy of the sign-in sheet and a summary of the evaluations that were completed.  These are necessary to satisfy record-keeping requirements for the Ohio Board of Nursing.  If you fail to complete this step, you will risk not being permitted to submit future requests for approval.
