If you completed and submitted a Security Affidavit and have received notification that access to ITS has been granted (you will receive a user ID and password via email), you will need to arrange to have ITS training with DODD MUI Unit. You may reach the unit at (614) 995-3810 or by email at MUI.Unit@dodd.ohio.gov. Training usually only takes a couple of hours.
If you do not have access to the ITS system yet, you will need to complete and submit a security request form by doing the following:
For County Boards:
Please follow the instructions listed to obtain and submit a security request form. Your Superintendent’s signature is required for approval. Please be sure to follow the instructions on the form – otherwise, it may be sent back to you!
For Providers:
Access the security request form here. Please print and complete the form and FAX it to (614) 995-3822. The MUI Unit Deputy Director’s signature is required for approval. Please be sure to follow the instructions on the form – otherwise, it may be sent back to you.
