PROVIDERS WHO USE NATIONAL CITY BANK FOR DIRECT DEPOSITS:
As part of their transition to PNC Financial Services Group, National City Bank will be providing updated account & routing information to Shared Services.
There should be no problems continuing direct deposits. Any questions regarding your account information should be directed to Shared Services at 614.338.4781, or to your local bank.
ATTENTION PROVIDERS:
The Office of Shared Services, a division of the Office of Budget and Management, will be sending out IRS form 1099's by January 31, 2010. If you made over $600 during 2009, you should receive a form 1099, unless you are classified as an agency. If you do not receive your IRS form 1099, need to request a duplicate, or have a question about what is on your 1099, please contact the Office of Shared Services directly. They can be reached by phone at (614) 338-4781 or 1-877-OHIOSS1 (1-877-644-6771), or by email at vendor@ohio.gov.
Please note that the Office of Shared Services must have your valid mailing address, and if a 1099 is returned to OSS for any reason, you will be inactivated in the State Accounting System. If you are inactivated, you will not receive payment. It is your responsibility to maintain a current mailing address with the Office of Shared Services, as well as the Ohio Department of Developmental Disabilities. If you need to update your mailing address, please fill out a vendor information form and fax it to Certification Support at (614)728-7836.

The Information Technology division has replaced the security affidavit form with a more user-friendly application. Instead of the Adobe PDF form, an on-line 'wizard' will help guide you through filling out the necessary information to apply, change, or terminate access to our on-line systems.

You are still required to accept the confidentiality agreement and you will be prompted to print out the form to be signed and either scanned, faxed, or mailed to DODD. Scan and email to: security.support@list.dodd.ohio.gov
