Update to Provider Information
The webpage used to update provider information is now revised. Providers with log-on approval to the previous site may now access the provider self-service update page by going to: https://odmrdd.state.oh.us/apps
If you are a new user wishing to update your provider information, you will need to complete a security affidavit and fax it to 614.752.4673 -- Affidavits may be obtained from the 'Forms' link on the DODD homepage at http://mrdd.ohio.gov/forms/

If you have a User ID and password to update Provider information, please click here to proceed to the self-service application.
If you do not have an existing User ID and password or you have forgotten your password, the self-service application is experiencing problems. Please send an email to PCS Support to request assistance.
